Midazolam belongs to the group of sedatives known as benzodiazepines. It is short-acting. Since midazolam has only a minor effect on the cardiovascular system, it is also frequently used in anesthesia to calm patients before surgical procedures and to sedate them during the procedures. In addition, the anterograde amnesia (reduced memory) and anxiolysis (anxiety-relieving effect) of midazolam are exploited. Furthermore, the active ingredient can be used for a short time to treat sleep disorders.
Like all benzodiazepines, midazolam has a central effect. It exerts depressant, soporific, antianxiety, relaxant and anticonvulsant effects. This effect occurs because midazolam binds agonistically to the receptors for the neurotransmitter gamma-amino-butyric acid (GABA) in the brain and enhances its depressant effect by increasing the frequency at which the cells’ chloride channels are opened. For short-term treatment of sleep disorders, especially difficulty falling asleep, the recommended dose is 7.5 to 15 mg as a single dose. The dosage in elderly people should be lower.
The duration of treatment should be as short as possible (between a few days and a maximum of two weeks). Treatment should not be discontinued abruptly, but gradually. Due to the rapid onset of action, midazolam should be taken immediately before bedtime. It should be noted that the sensitivity of individual patients may vary and symptoms of overdose may occur.
Midazolam may be used during pregnancy only if there is a compelling indication. In animal studies, embryotoxic effects were observed as with other benzodiazepines. Information on the use of midazolam during the first two trimesters of pregnancy is not available. Use of high-dose midazolam in the last trimester may result in adverse reactions in both the mother and the fetus.
